Prevention

  • Once yearly blood work for all preventative visits, VO2 max testing, resting metabolic rate testing and DEXA scan and 4 weeks supply of continuous glucose monitor, all included in the plan.

  • Additional blood work and imaging at discounted rates via third party vendors if paying cash (can opt to use insurance).

  • Intravenous fluids, medication, vitamin infusions available at the clinic or at house at additional cost.

Medications

PLUS 1 Preventative visit Plan, every 3 months*

1st Visit:

Heart health, metabolic health, cancer screening, medication review.

2nd Visit:

Mental health screening, cognitive health, and diet/nutrition assessment. Continuous glucose monitor data review.

3rd Visit:

Body composition analysis (DEXA scan), physical metrics, endurance metrics, review of data from smart watches.

4th Visit:

Sexual Health, screening for sleep disorders, urinary health, GI health, visual and hearing testing.

*All preventative health visits can be done on the same day *3-4 hour visit* for members paying for yearly subscription.

    4. You could choose to cost share our monthly fees with your employees by deducting their portion from their paychecks (still pre-tax dollars for them).

  • No, there is no commitment. Membership can be canceled anytime with a 30 day notice.

  • Yes, we can accept all family members who are 18 years and older.

  • As internal medicine physicians, we exclusively serve patients aged 18 and above.

  • At this time, we can not accept Medicare or Medicaid patients. However, that may change soon.

  • Detecting and addressing issues in their early stages promotes employee well-being and peak functionality. Direct Primary Care has been shown to decrease health related absenteeism. Employees need less time off work since they can be seen via Telehealth, after hours or a house call to your business.

  • If you currently do not offer any health benefits. Enrolling patients in Chicago Direct Primary Care can be a huge benefit to your employees since it will cover 80-90% of their medical needs.
